Most major private insurance companies cover our products. 

 

The Plaid Daisy is a preferred provider for OSF Health Plans, including Care Advantage and Care Preferred.  We are also a preferred provider for United Healthcare. We would be happy to do the billing for unless you would prefer to pay up front and then have your insurance reimburse.

 

MEDICARE

 

The Plaid Daisy accepts Medicare and is required by law to file claims for individuals who purchase any items that are covered by Medicare.  As of January 1, 2004, Medicare will cover 6 mastectomy bras, as long as you have a doctors prescription.  Medicare will also cover 1 breast prosthesis and 1 leisure breast prosthesis every 2 years.
